What You Can Do.
Talk Openly with Your Team
- If you are having trouble sticking with your care plan, tell your team about it.
- If you feel your care plan is not working, speak up. Tell your team what is not working so together you can make changes if needed.
Take Care of Your Health
- Follow the healthcare plan you and your team have worked out. Make sure you understand how to follow the plan.
- Set goals you can reach. Once you begin to see results, you and your team can discuss adding new goals.
Be an Active Team Player
- Talk with your team about your health questions. Share the successes and the challenges you’ve had with health care in the past.
- Tell a team member about any recent hospitalizations or visits you have made to the ER. It’s also important to list all the prescribed or over-the-counter medications you’re currently taking.
- Tell your team how you feel about the care you are getting from them.
- Make sure to tell your other providers or healthcare facilities your personal clinician’s information when seeking care outside of Neighborhood Healthcare.

Each of our locations has different hours of operation. For more information on location hours, click here. Our Patient Navigation Center is available to assist you Monday through Friday from 7:30 am to 5:00npm at 833-867-4642.
What languages do you speak?
All Neighborhood locations offer services in English and Spanish. Other locations and providers offer services in Arabic, Kurdish, Mandarin, Taiwanese, Tagalog, Italian, and Gujarati. Do I need to make an appointment?
You can Schedule Online, call 833-867-4642, or log in to your Patient Portal. Our skilled and compassionate team will assist you with appointment scheduling, connect you to important specialty services, and answer your questions. Our patient navigation center call center is available Monday through Friday, 7:30 a.m. to 5 p.m.
How do I make an appointment?
New patients can make an appointment by calling 833-867-4642. Existing patients can call or scheduling online. Same-day appointments are available on a first come, first served basis.
How do I cancel an appointment?
You can cancel your appointment by logging into your patient portal, calling 833-867-4642, or by text. To cancel by text, follow the instructions in your appointment reminder text message.

What is the Patient Navigation Center?
When you call 833-867-4642 you will get a member of our skilled and compassionate Patient Navigation Center team who can assist you to:
- Schedule your appointment
- Connect to important clinical, dental, and specialty services
- Answer medical records and referrals questions
- When you call Neighborhood Healthcare, our Patient Navigation Center is your first point of contact.
Does Neighborhood offer transportation?
Neighborhood Healthcare offers transportation assistance through Uber Health. If you need to schedule an appointment, but don't have reliable access to transportation, ask a staff member about Uber Health. Through this program, a staff member can schedule a ride to, and from, your appointment.
Who can receive health care at Neighborhood?
Neighborhood offers quality health services to every member of our community, regardless of situation or circumstance. We offer low-cost, discounted care for uninsured patients. We also accept most insurance plans, including Medi-Cal and Medicare.
What kind of payments does Neighborhood accept?
We accept cash, check, and all major credit cards. We offer flexible payment options and never turn anyone away due to an inability to pay. If you are uninsured or a self-pay patient, you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. For more information click here.
How do I make payment on an existing balance?
Patients can make a payment over the phone by calling the billing office at 760-737-6935 or by making a payment directly at any of Neighborhood’s sites.
What if I don’t have insurance?
We never turn anyone away due to a lack of insurance or inability to pay. If you don’t have health insurance, Neighborhood offers a Sliding Fee Discount Program and if eligible, your cost will be determined based on your income and household size. We can also help you apply for Medi-Cal and/or other assistance programs. If you are an uninsured or a self-pay patient, you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. For more information click here.
